NOTE: This website currently services the continental United States (48 states) ONLY.  If you live in Alaska, Hawaii, or have an APO or FPO address, or live in a foreign country, please contact qed@quod.com with the items you want and we will quote the appropriate terms and shipping charge.

Our minimum order is $15.00 (not including shipping).  Orders with a product total below $15.00 will not be processed.

• All blocks on the order form marked (•) MUST be completed or your order cannot be processed.

• Your "Bill To" address MUST be the same address to which your credit card statement is sent.  For YOUR protection, credit card companies require address verification for internet orders.  However, if you want your order sent to other than your credit card address please complete all the blocks in the "Ship To" column.  Since we ship exclusive by UPS a street address is REQUIRED for shipping. You do not have to complete the "Ship To" column if your order is going to the same address as appears in the "Bill To" column.

• There are 2 blocks for your address in case you want to include a company name or if you have the type of address that requires 2 lines.

• If you need to make any comments relative to this order, a space is provided.  All other comments should be sent via email.

• If you are a Rhode Island resident, indicate it in the box provided.  This will automatically compute the required Sales Tax for your order.  If you are not a Rhode Island resident, leave this box set on "Non RI resident".

• The shipping charge will be automatically calculated based on the weight of the items you have ordered.

• Once you have completed this form click on the "Continue" button.
